To request an urgent appointment for today or tomorrow (Monday to Friday)
- use our appointment request form, Monday to Friday from 8am to 6:30pm. We will respond within 2 working days during opening times
- visit the surgery and speak with a receptionist, Monday to Friday, when we are open
- phone us on 01642 827697, Monday to Friday, when we are open and a member of our team will help fill in the form with you

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

Routine appointments
To request a routine appointment in the next 7 days

Your appointment
However you choose to contact us we may offer you a consultation:
- by phone
- face to face at the surgery
- on a video call
- by text or email
Appointments by phone, video call or by text or email can be more flexible and often means the doctor or nurse can help you sooner.
Please give us as much notice as possible to allow us to offer the appointment to other patients.
To cancel your appointment:

- visit the surgery and speak with a receptionist, Monday to Friday, when we are open
We would like to inform our patients of a change to our service provision during the final 30 minutes of our core operating hours. To ensure that patients continue to have access to appropriate care during this time, the practice will offer alternative arrangements for care between 6:00pm and 6:30pm, by telephone 01642 944229 and from Park Surgery – Middlesbrough.
Elm Alliance Extended Access - Appointments available from 6pm to 9pm Monday to Friday and 9am to 5pm on Saturday. To book an appointment please phone reception on 01642 827697.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
